Job Description
Clinical Pharmacist
The Pharmacist is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. The Pharmacist will serve as a resource to the patients, physicians, pharmacists, nurses and other members of the health care team. Provides prescribed medications, drugs, and other pharmaceuticals, compounds medications, and prepares special intravenous solutions according to professional standards and practices. Assures safe and effective medication use and assures that drug storage is safe, controlled, and meets required conditions. Demonstrates proficiency in delivering care to assigned age specific patient population (e.g., infants, adolescents, adults, geriatrics, pediatrics), and participates in related continuing education.
Required qualifications:
PharmD
1 year Experience working as a pharmacist, preferably in an acute care setting
Required upon hire: California Pharmacist Registered
