    NURSE PRACTITIONER - FIELD

    Location: Clark County, Nevada

    COMPENSATION

    Base Salary - USD $108,108 to $136,000

    Full-time

    Company Vehicle provided for field visits.

     

    Required Work Shift: Monday thru Friday, 5 days a week, 8 am to 5 pm

     

    Must have current, unrestricted RN And NP licenses in the state of Nevada.

     

    Location: Field Based - This position requires driving and in-person visits to patient homes in Clark County, Nevada.

     

    Field Nurse Practitioner: Home-based care

     

    The Field Nurse Practitioner is responsible for providing in-person and virtual healthcare services to patients at home, in collaboration with the attending physician, through advanced assessment and identification of health states including abnormal conditions, diagnosis, treatment and evaluation. This team member will deliver a mix of home-based palliative care, post-acute, chronic disease and primary care management for homebound patients, and higher acuity “Hospital at Home” care to complex patients.  Responsible for collaborating with the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values.

     

    How will you make an impact:

     

    Utilize evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines, within the scope of practice, and under supervision of a collaborating physician

     

    Acquire patient health histories, performing physical examinations, ordering and interpreting complex diagnostics, and making critical decisions regarding a wide variety of health situations, including high risk and crisis situations

     

    Perform health assessments, specialized risk assessments, and developing plans of care for patients facing acute and chronic and health conditions.

     

    Make routine in-person (and virtual) visits to patient homes as clinically necessary and in accordance with Medicare guidelines

     

    Document encounters precisely and timely on the appropriate EHR system, and clearly communicating plans of care and/or any changes of condition to patients and team members.

     

    Participate in Rotating On-Call program which may include after-hours, weekend and holidays

     

     

    Minimum Requirements:

     

    Requires an MS in Nursing and minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.

     

    Current unrestricted RN license and NP license in the state of Nevada is required.

     

    Prescribing or furnishing authority/DEA licensure in good standing.

     

    For CareMore business units, satisfactory completion of a Tuberculosis test is a requirement for this position.

     

    Preferred Skills, Capabilities and Experiences:

     

    Experience in complex care, wound care, home health experience is a plus.

     

    2-5 years of experience working in an acute care setting such as ICU, CCU, telemetry, emergency medicine is a plus.

     

    Proficient in use of EMR and other technology use.

     

    Current, unrestricted NP license in the state of Nevada

    Current, unrestricted RN license in the state of Nevada

    DEA License

    IDEAL CANDIDATE

    Must be Open to working Monday thru Friday, this is what is Required for this position.

    Experience in working with Geriatric patients.

    Open to Daily Local travel visiting patients in Clark County, Nevada.

    Must have a current, unrestricted NP And RN licenses in the state of Nevada.

    DEA license highly preferred.
