    Our hospital client is calling a Lead Clinical Pharmacist - Inpatient (0.9 FTE, Variable/Rotation) to Memorial Hospital . This position will workdays and evenings (4 9-hour shifts) starting as early as 6am and ending as late as 7:30pm. In addition, you will be expected to work 1-2 weekends per month (both Saturday and Sunday) from 7:30am to 5pm. You will also be expected to be on-call for nights 2-3 shifts per week. When you work weekend shifts, Saturday and Sunday will be your on-call nights.

    We are seeking a Lead Clinical Pharmacist - Inpatient that will be coordinating the daily organizational and operational activities of the Central Pharmacy. Acts as immediate supervisor to pharmacists and technicians involved in medication distribution. Maintains and adjusts staffing as required per workload/staff availability.


    In this position you will have the following responsibilities:

    Provides quality patient care in an efficient, caring manner. Adapts to fluctuating workload. Carries fair share of workload. Utilized non-patient care time to maintain department. Needs little or no direction in accomplishing daily work.

    Drug orders are processed accurately in a timely manner. Utilizes information systems timely and accurately. Properly documents clinical interventions and drug information requests.

    Reviews patient records and orders regarding drug therapy taking into consideration age and disease state. Recommends and initiates changes as appropriate; is successful in improving patient outcome through monitoring, intervention and patient education consistently.

    Initiates and effectively utilizes problem solving skills to promote a positive work environment and interpersonal relationships, while maintaining professionalism and confidentiality.

    Attends 80% of staff meetings. Maintains current knowledge of memos distributed and relevance to daily operations. Maintains education/in-services file current. Meets department and hospital education/in-service requirements. Adheres to hospital policies and procedures as well as state and federal laws.

    Reviews and interprets physician's orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and co-morbidities. Checks for drug incompatibilities and allergies, and take appropriate action when necessary. Reviews patient records and orders regarding drug therapy, recommends and initiates changes that are consistent with the patient's age; is successful in improving patient outcome through monitoring and intervention consistently.

    As required, regularly consults with physicians and nursing staff members concerning the use, availability, dosage form, side effects and possible incompatibilities of new as well as currently used drugs; always take time to clearly explain information to concerned patients.

    Follows proper computer order entry procedures.

    Responsible for the completion of all assigned work procedures during the shift, while maintaining the highest standards of quality.

    For medications defined as part of clinical services (DUE, pharmacokinetics), concurrent interventions and follow-up will be made in order to assure the appropriateness, safety, and efficacy of the medication.

    Supervises and reviews work performed by supportive personnel.

    Uses pharmacokinetic principles in establishing patient specific dosage regimens as appropriate and effectively documents all clinical monitoring.

    Understands the principles of the hospital formulary and follows formulary procedure.

    Maintains current awareness of and complies with the Quality Management programs, including awareness of current monitors, and participates as indicated.

    Documents all clinical interventions and workload statistics.

    Acts as coordinator for daily work flow within the Central Pharmacy.

    Coordinates staffing and maintains efficient, to include managing staffing shortages and periods of diminished workload.

    Maintains excellent communication with the nursing floors and physicians related to medication management and delivery service.

    Assists in meeting regulatory requirements.

    Represents department in team/committee activities as requested.

    Participates in departmental management meetings.

    Evaluates performance of Central Pharmacy staff.

    Oversees Central Pharmacy orientation and training for new employees, residents and students



    Required qualifications for this position include:

    Current or eligible for licensure as a Registered Pharmacist in the state of Oregon.

    Two (2) years of hospital pharmacy experience as a licensed pharmacist.

    Preferred qualifications for this position include:


    Completion of a pharmacist residency (PGY1 and/or PGY2)

    Base Salary - USD $123,000 to $156,000
