We are seeking candidates for a Director of Case Management role for a client hospital in Northwestern AZ Hospital.
REPORTS TO Chief Nurse Officer
The hospital serves the dynamic area surrounding Bullhead City, Arizona, Laughlin, Nevada, and Needles, California. It is conveniently located in Ft. Mohave, a Bullhead City suburb. In fact, the hospital is located several miles due east of the exact spot in the Colorado River where the three states come together!
Valley View opened in November 2005 as a 102,000-square-foot acute-care facility with two adjacent medical office buildings. Growth in the area meant growth at the hospital, and Valley View’s main building is now 126,000 square feet. This state-of-the-art hospital is currently licensed with 52 medical/surgical beds, 12 acute rehabilitation beds, 12 intensive care beds, and an eight-bed labor, delivery and post-partum unit, and is the only all private bed hospital serving the Bullhead City/Laughlin/Needles area. In addition to private rooms, all admitted patients have individual restrooms (including showers), cable TV and free local newspapers, and some rooms have recliner loungers for family members.
Salary range
$110,000.00 to 135000 Plus
25,000 sign-on bonus/relocation
RELOCATION PACKAGE

Bachelor’s degree Required.
Certifications: BLS required upon hire
5 years in management experience in acute short-term hospital/case management
Arizona Registered Nurse and/or Compact State Registered Nurse Required
Nice-To-Haves
Case Management Certified
Master's degree preferred.
Job Description
Position will provide ongoing support and expertise aligned with strategic plan to Valley View Medical Center through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and management of discharge planning. Accountable for the care, coordination and discharge planning of all patients.
Minimum Education
Graduate of a Program in Nursing at Bachelor of Science level or higher required.
Master's degree preferred
CCM or ACM Certification preferred
Minimum Work Experience
Minimum of five (5) years management experience, five (5) years of utilization review / case management experience in an acute short term hospital, five (5) years experience in discharge planning in an acute short term hospital. Requires experience with Interqual or MCG criteria software, advanced computer skills and is proficient in Microsoft Word, Excel and PowerPoint. Experience preferred with MedHost EMR and with MorCare
Current RN Licensure from the State of Arizona or compact license accepted by State of Arizona.
Certifications: Basic Life Support (BLS
