Director of Case Management –
Salary: $83,595 - $109,439 (10 years) - $ 119,185 (Max 20 years) - Salary is based on years of experience. Pay Grade Counts Years of Leadership Only
Director Bonus: upto 15% of Salary - Based on performance
Sign on Bonus: upto $5,000 - with a 2 yr commitment
Relocation: $3,000 ( Depending on distance)
Our client is an award-winning 164-bed acute care hospital located in the heart of northeast Tarrant County in North Richland Hills. The hospital has over 300 physicians and 30 medical specialties. The ER has about 39,000 visits each year and is a Level III Trauma Center. Additionally, we are a Primary Stroke Center, perform spine surgery and joint replacements and have a comprehensive cardiovascular services department. Our client hospital is an accredited Cycle 5 Chest Pain Center with a cardiac rehabilitation program.
Position Summary
The Director of Case Management provides daily leadership for case management services, including social services and discharge planning. Enhances clinical and financial outcomes through performance improvement activities. Assists department in regulatory compliance.

• Provides daily leadership for case/resource management to achieve maximum deployment and integration. Measures such functions throughout the organization using the facility improvement process.
• Use data analysis to drive improvement projects.
• Works cooperatively with physicians to achieve the utilization/case management plan.
• Collaborates with clinicians and support staff in designing and implementing processes to comply with regulatory requirements Assists with JCAHO survey readiness/preparation

Experience
2 + years RECENT hospital experience in Case Management
MUST HAVE ACUTE CARE HOSPITAL EXPERIENCE
MUST have leadership experience
Licensure/Certifications:
• Texas State RN license or compact license is accepted
•Certification in Healthcare Quality and/or Hospital Case Management, Preferred

• Salary range $95,000 - $115,000 (20 years of experience to be considered for that rate for the max)
• Bonus eligible up to 15% of base annual
• SOB: Up to $10k
• Relo: up to $12k

Job Summary
The Case Management Director follows the hospital's Case Management Program that integrates the functions of utilization review, care coordination, discharge planning, and resource management into a singular effort to ensure, based on patient assessment, care is provided in the most appropriate setting utilizing medically indicated resources. The hospital’s case management model outlines a collaborative practice to improve quality through coordination of care impacting length of stay, minimizing cost, and ensuring optimum outcomes.
The Case Management Director is a member of the leadership team and plays an active role in the execution of, and success of the facilities strategic business plan.
Job Responsibilities
• The Case Management Director is responsible for the organizational leadership of the case management department in support of the hospital mission, vision and strategic goals.
• The director is responsible for strategic planning of the department while managing the day-to-day oversight of the department including but not limited to: payroll, hiring, counseling, mentoring/coaching, scheduling, orientation, competency validation of new and existing staff, and annual evaluations.
• The Case Management Director is responsible for compliance with CMS Conditions of Participation regarding Utilization Review, Care Coordination, and Discharge Planning including implementation and annual review of the Utilization Management Plan and assisting with the coordination of the Utilization Management Committee.
• The Case Management Director is also responsible for ensuring that Joint Commission’s Provision of Care regarding discharge planning and patient needs for care, treatment, and services after discharge or transfer are met.

• Post Graduate Degree, Preferred
Experience
• 2 – 4 years’ experience in a Director or Manager position
• Minimum of 3-5 years of acute care hospital or clinical experience in case management
• Must have Acute Care Hospital experience.
• Good candidate would be someone who has Manager level experience in a large hospital who is ready for a Director position in a smaller hospital
Licensure/certification/registration:
• RN License in the State of Texas or compact license, Required
