Two Years of Hemodialysis Clinical Experience
Responsible for the Initial Assessment of all new patients for appropriateness of admission using Comprehensive Interdisciplinary Assessment, Plan of Care and medication review.
Provides and coordinates care of the hemodialysis patient using nursing process including care given during pre-dialysis, intra-dialysis and post dialysis procedures.
Coordinates the development and evaluation and revisions or the individual plan of care with the interdisciplinary team for the ESRD patient.
Acts as discharge nurse by completing nursing discharge summary and assisting with transfer orders as needed.
Understands water treatment system and performs quality control.
Cleans and prepares hemodialysis equipment and solutions.
Leads educational/development activities for staff meetings and daily briefs. Provides input to manager on performance evaluations for staff members in the unit.
Identifies and resolves issues affecting the delivery of clinical services.
Performs performance improvement studies such as audits and assists with implementation of PI initiatives.
Serves as a resource to staff and external contacts on issues related to patient care, patient flow and clinical standards, policies and procedures. Helps on the floor during busy times and switches to tech/floor RN role when needed.
Monitors departmental compliance with applicable requirements, including accreditation, legal, and The Joint Commission.
Responsible for the evaluation of ESRD clinical performance monitoring (CPM) as outlined in the Measures Assessment Tool (MAT).
Reviews all lab reports implementing physician protocols related to monthly lab monitoring including: anemia management, iron management, mineral bone disease, transplant modality, and access monitoring. Involved in setting up patients for transplant evaluations and any procedures/appointments needed to set on the transplant waitlist.
